If your application is accepted*, you will receive via email a Provider Agreement outlining the terms and conditions that will govern your relationship with Canopy. The email will provide instructions for how to accept the terms and conditions of the agreement electronically (or opt to submit a physical copy of the agreement if you prefer).

* You may be selected for further screening before being accepted to the Canopy network. If so, you will be contacted by a Canopy representative who will walk you through a few additional requirements, which may include a visit to your place of business/practice. Note: Canopy reserves the right at its sole discretion, to deny any provider access to its network or request additional information as part of its provider screening process.
